Technology Forms
Parents: Students are bringing home 3 technology forms that must be completed and returned to school on Monday, August 10th, in order for students to receive their school-issued computers! These forms are:
1. Technology Insurance Form and $25
2. Student/Parent Training Form
3. Acceptable Use Form.
Students will NOT be issued devices unless all forms are completed, signed and turned in. Forms are also available on the FMS website.
Students who already have their devices from Summer School need to start bringing them to school on Monday. These students will not need to complete the forms again, unless they will not filled out this summer.
